to register by mail:

  • Download and print two copies of the 1601 form (form available here).
  • Complete and sign both copies.
  • Include a readable copy of the player's birth certificate if they were not registered in 2007 or 2008.
  • Include the appropriate fee. Checks made payable to 'SVYSA'. (Please do not mail cash)

Mail completed packages to: SVYSA Registrar, POB 1992, Sonoma, CA 95476. Completed mail-in packages must be postmarked by May 31. Incomplete packages will delay registration and possibly result in late fees.
